9a44c012ba Fix crashes when dynamically removing input type=file elements by not calling GetValue() during Destroy() since that re-adds the frame to the primary frame map. Bug 203041, 238906, patch originally by mats.palmgren@bredband.net, updated by me, r+sr=bzbarsky.

b5347dd60c Change all callers of nsIPresContext::GetShell() to use the non-refcounting versions. Most callers now use PresShell(), a few that were checking for null conditions are using GetPresShell() -- this will be remedied once we can ensure that a prescontext never has a null pres shell.
