b5347dd60c Change all callers of nsIPresContext::GetShell() to use the non-refcounting versions. Most callers now use PresShell(), a few that were checking for null conditions are using GetPresShell() -- this will be remedied once we can ensure that a prescontext never has a null pres shell. 1c89787dc0 fix for bug 206379: 
- combine nsICharsetConverterManager2 and nsICharsetConverterManager
- get rid of nsIAtom in most of the methods
- provide versions of getUnicodeDecoder/Encoder which don't do alias resolution
- change all charset types to ASCII strings
- clean up some other i18n APIs which could be simplified
- fix all consumers of all changed i18n interfaces

51b17b5bf0 Bug 88413: Remove |GetUnicode()| from nsString (and replace it with |get()|). r=dbaron, rs=scc. 
This removes all call-sites I can currently fix. Tomorrow I'll try to get someone to checkin my changes to security/ and I'll get some help with the Netscape side of things.

nsString::GetUnicode()'s final death-blow will be dealt soon. Please keep this in mind as you add new code :-)

161f586d22 Bug 77834: Fix problem where form control state is being restored to the wrong form control or not restored at all when going back and forth through session history. The problem was that the hash key we used was the content ID, which was not consistent from page load to page load.
