Move all Python initialization code into a new global

PyXPCOM_EnsurePythonEnvironment. At the same time remove the old, dead
code behind PYXPCOM_USE_PYGILSTATE and LOADER_LINKS_WITH_PYTHON.
Not part of the default build.
