dda6ff3c99 Bug 348748 - Replace all instances of NS_STATIC_CAST and friends with C++ casts (and simultaneously bitrot nearly every patch in existence). r=bsmedberg on the script that did this. Tune in next time for Macro Wars: Episode II: Attack on the LL_* Macros.

ae9e02402a deCOMify nsIPresShell::GetRootFrame. Change callers inside of gklayout to use FrameManager()->GetRootFrame() instead so that the call can be inlined. Bug 253889, r+sr=roc.

468d3e2e8d Eliminate nsIStyleSet as an XPCOM interface, making the implementation internal to layout. Frame constructor ownership moves to the pres shell, and the style set methods that forward to the frame constructor are gone. Rewrote style set sheet add/remove to reduce code size.
