1d16197560 Attempt to fix Mac clobber build bustage by getting rid of a bogus MANIFEST installation and changing the way a header is generated in the hope that it will keep the Mac build automation from getting confused (bug 133999). Patch from peterv (r=dmose), with tweaks by me (r=peterv); sr=sspitzer; a=jelwell (the sheriff)

df343ef883 Changed day / week views titles to act like the month view's title. Removed the extra years in the month view as it was just too confusing.

daf49afcf3 finally have month view in decent working order (for resizing) 
Fixes bug:
110057 Month view does not fit in (smallish) browser area
Partially fixes bug:
125366 Calendar does not maximize to window size (too big)
(Month view only is fixed -- breaks week & day views temporarily (it doesn't really break them, it just makes them look bad for now))

866bc07188 Bugzilla bug 132208: added the 'tcp_nodelay' field to Linux's _MDFileDesc, 
which is set by pt_SetSocketOption or pt_Accept and used by
pt_LinuxSendFile to avoid a setsockopt(TCP_CORK) call that is bound to
fail.  Modified files: _unixos.h ptio.c
